What information we collect

  • Enquiries: your first and last name, date of birth, email, mobile number, which assessment you're interested in, and, if you choose to tell us, how you heard about us, good times to call, your message and whether you'd like to hear about children's assessments. We ask for your date of birth to check an assessment is suitable for you and to set up your client record if you go ahead. Please don't include medical details in the enquiry form.
  • Prospective clinical associates: if you register interest in joining our associate panel, we collect your contact details, professional registration, experience, availability and, if you choose, your CV. We use this only to discuss associate opportunities with you (legitimate interests). It's stored in our Google Workspace account and deleted after 12 months if we don't go on to work together, or sooner if you ask.
  • Assessment information: the information in your pre-assessment forms, interviews and sessions, including information about your health, development and history. This is "special category" data, and we treat it with extra care.
  • Informant information: the name and contact details of anyone who takes part as an informant, and what they tell us about you.
  • Payment records: what we need to take payment and keep financial records.

The self-screeners on this website run entirely in your browser. Your answers are never sent to us or stored.
